Coordinated Care Organizations (CCOs)

Advanced Health serves all communities in Coos and Curry Counties. We proudly connect members to high quality Medical, Dental, Behavioral Health, and Substance Use Treatment through local collaboration and innovation.

We are dedicated to making a difference in the health and well-being of our members and all communities served.

If your project aligns with one of the CHIP (Community Health Improvement Plan) priorities, we invite you to submit an application:

CHIP Workgroups:

Community Partners working on a project that aligns with a CHIP Focus Area:

Please fill out the Curry CHIP Reporting Form so we can learn successes and challenges toward reaching your goal. The report can be filled out as a progress report or a final report, depending on the timeline of your project. 

Community Advisory Councils

The Coordinated Care Organizations (CCOs) that serve Curry County each have a Community Advisory Council (CAC). The CAC advises it's CCO on community health needs and how to achieve better health and better care throughout Curry County, while lowering costs.


The CAC’s role is to make sure that the healthcare needs of the people in our community are being addressed. Most CAC members are people who live in Curry County and are covered by the Oregon Health Plan. Other council members are representatives of community agencies.
